Progressive 350+ Bed Acute Care Hospital located near Grand Rapids, Michigan is currently recruiting for a full-time Clinical Pharmacist. The selected individual will:* Serve as the dedicated Clinical Pharmacist to support the kidney transplant program for recipients and living-donors at our facility* Provide support during all phases of transplant including discharge planning* Attend and participate in inpatient rounding/planning/review with multidisciplinary team*Attend and participate in all selection meetings*Provide general Clinical Pharmacy services to non-transplant patients in support of the clinical services team for the Department of Pharmacy. (Approximately 70-80% of practice in Kidney Transplant and 20-30% of clinical practice in other inpatient areas.)Education/Training Requirements: Doctor of Pharmacy degree from ACPE-accredited institution Completed a Pharmacy Practice (PGY-1) Residency Board Certified or Board-eligible in Pharmacotherapy Completed a Specialty (PGY-2) Residency in Solid Organ Transplant OR 3 or more years' experience in solid organ transplant including kidney transplantation for those without a PGY-2 Residency Experience as a Clinical Pharmacist Transplant center experience, preferredLicense/Certifications: Licensed pharmacist in Michigan (or eligibility to be licensed in Michigan) BLS/ACLS certification (or completed upon hire)DutiesClinical pharmacy services will be provided to the transplant recipients and/or the family/caregivers of transplant recipients of kidney transplants during the pre-, peri-, inpatient, discharge and post-phases of transplantation and donation (where applicable for living donors). The transplant pharmacy service will include identifying, solving and preventing medication related problems in the kidney transplant population and living donors for the kidney program. The transplant pharmacist will provide education regarding the safe and effective use of medications in the post kidney transplant population to the healthcare team, patients and families. The transplant pharmacist will maintain documentation related to transplant pharmacy activities that are complaint with CMS regulations. This position will also maintain competency in more broad clinical pharmacy areas in order to provide support to the acute care clinical pharmacy services team.
